I've spent half the day troubleshooting this, with very odd results. E.g., the data in the "Email" fields in the dozen or so previous submissions never got saved to the database (although everything else did) – but when I fixed a problem with our mailserver (for some reason, SpamAssassin was having a problem with these form submissions), that problem "magically" fixed itself. Everything works now, but I don't like magic, so I don't trust it much.
Just as a save-yourself-a-problem feature, you might want to add a "debug" option to Formidable in the future (e.g., "here's what got saved to the DB, and here's what got handed to the mail routine.") Seems like it would prevent people from posting here if the problem isn't actually coming from Formidable… :)
